RNCM Job
Summary:
Provide direct palliative care to terminally ill patients; support and provide education to their families while functioning within a medically approved plan of care.
This is a position that will support areas of Aurora, Parker, and Castle Rock as needed.
Qualifications/Educational Requirements:
Must be a currently licensed RN in the State of Colorado.
Must have demonstrated knowledge and experience in the art and science of pain and symptom management and have well-developed physical assessment skills.
Responsibilities/
Essential Functions:
Assess patient referrals for appropriateness of hospice care and admit new patients timely with proper documentation.
Responsible for patient care scheduling.
Teach patient and family about realistic expectations of disease and dying process.
Calm, non-anxious presence and support.
Assess and monitor pain; evaluate the pain management's effectiveness.
Complete paperwork and charting correctly, timely, and appropriately for the needs of Medicare, State Health Department, ACHC, and Dignity standards.
Responsible for assisting in marketing and cultivating strong relationships with community staff, with an expectation to grow business in communities where you have patients.
Observe and assess patients through such data sources as weight, pedal edema, patient history and input, shortness of breath and respirations.
Administer medication and manage symptoms.
Monitor the patient's status and report to the physician.
Teach caregivers and nurses how to care for a loved one with end-stage disease.
Present hospice philosophy and services; explain patient rights and responsibilities.
Other interventions based on patient/family needs.
Working Conditions:
General offices, long-term care facilities, assisted living facilities, and patient home settings will be the primary locations of the Hospice RNCM activities.
Exposure to infectious diseases is possible and universal precautions should be taken at all times.
Travel is a requirement of this position.
Physical Demands:
Position may require occasional physical exertion.
Physical requirements of the Hospice RNCM may include, at varied frequencies:
sitting, stationary standing, walking, general mobility, crouching, kneeling/crawling, stooping (bending at waist), twisting (knees/waist/neck), turning/pivoting, climbing steps, balancing, reaching overhead, reaching extension, grasping, pinching, pushing, pulling, lifting, carrying, and lifting up to 50 lbs.
Must be able to travel to businesses, offices, facilities, or patient homes where conditions may be adverse due to weather, environment, or patients' lifestyles.
Scheduling and Case Loads:
Hospice RNCM are expected to work two (2) on-call holiday shifts per year with a shift differential/premium.
Hospice RNCM can expect a caseload of 15-18 patients.
Continuing Education Requirements:
Employee is required to participate in 12 hours of continuing education per year.
Most required in-services can be joined remotely/virtual.
Training:
Even the most experienced Hospice RNCM will receive a minimum of 2 weeks of paid orientation and shadow days in the field including 1:
1 time with clinicians from all disciplines.
The average orientation period before independent patient care is assigned to the Hospice RNCM is 30 days.
Orientation is tailored to the individual.
Compensation:
Includes a salary of $38.
00 to $44.
00 per hour depending on experience, shift differential for on-call, plus mileage and company-provided cell phone.
Dignity Hospice pays 90% of the employee premium towards the base health insurance plan (United Healthcare), dental, and vision.
