9395 Crown Blvd Parker, Colorado 80138 ? The role you'll contribute:
Plans, implements, and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients, linkage to resources in the community.
The value you'll bring to the team:
Assess patient's physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data.
Evaluates patient's response and intervenes to ensure optimal patient outcome.
Verbalizes an understanding of the value of measurement and the importance of continuous quality improvement data.
Use quality measures to improve performance and accountability for patient outcomes, patient experiences and safe delivery of care.
Participate in unit-based Quality improvement, evidence-based practice and research activities.
Use current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues.
QualificationsThe expertise and experiences you'll need to succeed:
Required:
Master of Social Work (MSW) Cardiopulmonary Resuscitation (CPR) Current Basic Life Support (BLS) from American Heart Association for facilities with an Automated External Defibrillator (AED), preferred for facilities without an AED.
Current Driver's license Licensed Clinical Social Worker (LCSW) Preferred:
6 months home care/hospice experience 2 years post master's experience in health care or related field This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
Recommended Skills Advanced Cardiovascular Life Support (Acls) Automated External Defibrillator Basic Life Support Cpr Clinical Works Continuous Quality Improvement (Cqi) Estimated Salary: $20 to $28 per hour based on qualifications.
